His chest X-ray film at the annual health check-up in April 1993 showed bilateral infiltrative shadows. The abnormal shadows were enlarged on the next X-ray film (May 26). Laboratory examinations showed blood eosinophilia, marked elevation of serum IgE concentration and lgE antibody to Aspergillus. Bronchofiberscopy revealed mucoid impactions at the orifices of the right B^9 and left lower lobe bronchus. Pathological examinations revealed bronchial inflammatory change with eosinophils. The sputum culture showed Aspergillus niger. Although the clinical features and laboratory finding of this case did not satisfy Rosenberg's criteria, we diagnosed this case the early stage of ABPA (ABPA-S). ABPA is potentially fatal because it will induce fibrotic lung destruction. Early diagnosis and treatment before the development of irreversible damage are necessary to prevent the chronic respiratory failure. In this case, we were able to find at relatively early stage without central bronchiectasis.
