We assessed whether either approach would facilitate successful catheter placement in the paravertebral space.DESIGN: Randomised controlled trial.SETTING: University hospital, July 2015 to March 2016.PATIENTS: Sixty patients scheduled to undergo thoracotomy were randomly allocated into two groups.INTERVENTIONS: A microconvex probe was placed transversely between adjacent ribs (transverse) or sagittally between adjacent transverse processes (parasagittal). When the Tuohy needle reached the paravertebral space, a catheter was inserted to a depth of 4 cm. Then, 0.5-ml radiocontrast was injected through the catheter under fluoroscopy.MAIN OUTCOME MEASURES: The primary outcome was successful catheter placement in the paravertebral space; secondary outcomes were 0 to 100 mm visual analogue scale pain score and morphine consumption in the first 24 h.RESULTS: All patients received the allocated paravertebral block. Correct catheter placement occurred in 23 (77%) and 24 patients (80%) using the transverse (n=30) and parasagittal approaches (n=30), respectively (P = 1.00). Five patients were excluded due to changes in surgical procedure. Postoperative pain, represented by median [IQR] visual analogue scale score, was 19.5 [12 to 25] at rest and 55 [44 to 77] on movement with the transverse approach (n=28) vs. 22 [12 to 33.5] at rest and 59 [41.5 to 75] on movement with the parasagittal approach (n=27) (P = 0.57 at rest, P = 0.76 on movement). Median morphine consumption was 11.5 [5 to 21] and 11 [5 to 18] mg in the transverse and parasagittal approaches, respectively (P = 0.99).CONCLUSION: There were no clinically significant differences between approaches for continuous ultrasound-guided thoracic paravertebral block using a microconvex probe, and both approaches achieved a high rate of correct catheter placement.TRIAL REGISTRATION: UMIN Clinical Trials Registry identifier: UMIN000015988.","subitem_description_language":"en","subitem_description_type":"Abstract"}]},"item_10_publisher_32":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"Wolters Kluwer Health","subitem_publisher_language":"en"}]},"item_10_relation_11":{"attribute_name":"DOI","attribute_value_mlt":[{"subitem_relation_type":"isVersionOf","subitem_relation_type_id":{"subitem_relation_type_id_text":"https://doi.org/10.1097/EJA.0000000000001223","subitem_relation_type_select":"DOI"}}]},"item_10_select_15":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_select_item":"author"}]},"item_10_source_id_61":{"attribute_name":"ISSN(print)","attribute_value_mlt":[{"subitem_source_identifier":"0265-0215","subitem_source_identifier_type":"PISSN"}]},"item_1615787544753":{"attribute_name":"出版タイプ","attribute_value_mlt":[{"subitem_version_resource":"http://purl.org/coar/version/c_ab4af688f83e57aa","subitem_version_type":"AM"}]},"item_access_right":{"attribute_name":"アクセス権","attribute_value_mlt":[{"subitem_access_right":"open access","subitem_access_right_uri":"http://purl.org/coar/access_right/c_abf2"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"Fujii, Tasuku","creatorNameLang":"en"}],"nameIdentifiers":[{"nameIdentifier":"103397","nameIdentifierScheme":"WEKO"}]},{"creatorNames":[{"creatorName":"Shibata, Yasuyuki","creatorNameLang":"en"}],"nameIdentifiers":[{"nameIdentifier":"103398","nameIdentifierScheme":"WEKO"}]},{"creatorNames":[{"creatorName":"Shinya, Sonoe","creatorNameLang":"en"}],"nameIdentifiers":[{"nameIdentifier":"103399","nameIdentifierScheme":"WEKO"}]},{"creatorNames":[{"creatorName":"Nishiwaki, Kimitoshi","creatorNameLang":"en"}],"nameIdentifiers":[{"nameIdentifier":"103400","nameIdentifierScheme":"WEKO"}]}]},"item_files":{"attribute_name":"ファイル情報","attribute_type":"file","attribute_value_mlt":[{"accessrole":"open_date","date":[{"dateType":"Available","dateValue":"2021-02-09"}],"displaytype":"detail","filename":"Manuscript_Eur_J_Anaesthesiol_2020_nagoya-u.pdf","filesize":[{"value":"1.1 MB"}],"format":"application/pdf","licensetype":"license_note","mimetype":"application/pdf","url":{"label":"Manuscript_Eur_J_Anaesthesiol_2020_nagoya-u","objectType":"fulltext","url":"https://nagoya.repo.nii.ac.jp/record/31326/files/Manuscript_Eur_J_Anaesthesiol_2020_nagoya-u.pdf"},"version_id":"0ef5be60-da1f-4b24-8a83-9ec253818298"}]},"item_language":{"attribute_name":"言語","attribute_value_mlt":[{"subitem_language":"eng"}]},"item_resource_type":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"resourcetype":"journal article","resourceuri":"http://purl.org/coar/resource_type/c_6501"}]},"item_title":"Transverse vs. parasagittal in-plane approaches in ultrasound-guided paravertebral block using a microconvex probe","item_titles":{"attribute_name":"タイトル","attribute_value_mlt":[{"subitem_title":"Transverse vs. parasagittal in-plane approaches in ultrasound-guided paravertebral block using a microconvex probe","subitem_title_language":"en"}]},"item_type_id":"10","owner":"1","path":["501"],"pubdate":{"attribute_name":"PubDate","attribute_value":"2021-02-09"},"publish_date":"2021-02-09","publish_status":"0","recid":"31326","relation_version_is_last":true,"title":["Transverse vs. parasagittal in-plane approaches in ultrasound-guided paravertebral block using a microconvex probe"],"weko_creator_id":"1","weko_shared_id":-1},"updated":"2023-01-16T04:24:49.093219+00:00"}
