成人女性を対象に膝蓋大腿関節痛を有する症例と疼痛を有しない健常者の膝蓋骨可動性を比較し、膝蓋大腿関節症症例における膝蓋骨可動性の特徴を検討することを目的とした。【背景】 膝蓋骨可動性の異常はその原因の一つとされているが、現在それを証明する報告は十分にはなされていない。これは、信頼性のある臨床的な膝蓋骨可動性計測方法が確立しておらず、客観的に膝蓋骨の異常可動性が確立されていないことに由来する。【方法】 対象は健常女性22 名22 膝(コントロール群)と膝蓋大腿関節痛を有する成人女性22 名22 膝（PFP:patellofemoral pain 群）とした。PFP群に対応するコントロール群は、年齢（±３歳）、身長（±5%）、BMI（±4%）をあわせた。膝蓋骨可動性計測には定量的に計測可能な特別な計測器を用いた。内側・外側膝蓋骨可動距離、膝蓋骨可動性のバランス（外側－内側膝蓋骨可動距離）、内側及び外側膝蓋骨可動距離を膝蓋骨幅で除した指標を用いた。【結果】 両群間で膝蓋骨可動性に関する計測値には有意な差が認められなかった。膝蓋骨可動性の正常範囲をコントロール群の平均±２SD とすると、外側膝蓋骨可動距離は7.2±17.6ｍｍであり、内側膝蓋骨可動距離は6.8±14.0ｍｍであった。本膝蓋骨可動距離計測方法の検者内、検者間の級内相関係数は、0.80 から0.97であった。【結語】 膝蓋大腿関節痛の有無により有意な膝蓋骨可動性の差は認められなかったが、本研究で仮定した正常範囲を逸脱した症例がPFP
